PREREQUISITES AND PLACEMENT IN THE CURRICULUM: Medicine Core Clerkship.
PURPOSE: The clinical objectives are for the student to understand the presentation, evaluation, diagnostic testing and therapy of the critically ill adult.
COMPETENCIES: There are no formal technical skills expected of the medical student; however, the student who is interested in performance of arterial line insertion, central venous catheterization, or other diagnostic procedures may be allowed to assist and perform them.
INSTRUCTIONAL FEATURES: A one month ICU senior student rotation will be
offered through ALGH to medical students, and will be a closed unit experience
in our ICU. A student will be assigned
to two of the teams that currently take care of patients in the ICU. A team
consists of up to 5 senior residents, two interns, and one medical student. The
student will see patients of either intern on his/her team to allow for a
greater patient mix. Students will be expected to work up approximately four
new patients per week and carry no more than eight patients on their service.
The students are expected to pre-round with their interns and seniors. They
will then present their patients on multi-disciplinary rounds to the attending
on service. They will be allowed to follow their patients from the ICU to the
step down units located on the same floor. Patients span the spectrum of
medical illnesses. Medical students will have two days off per week. Students
will also have a separate didactic teaching session with Arvey Stone, MD, once
a week. During this time they will be expected to present cases and be able to
discuss management with Dr. Stone. A core of ten lectures have been selected
for the students and interns while on the ICU rotation. These lectures will
include: Ventilator Support, Acute Renal Disease, Fluid Balance, Electrolytes,
Infectious Disease, Neurological Emergencies, Acute GI Bleeding, Acute Cardiac
Care, Acute MI, Hemodynamics, Arrhythmias, Hematological Problems in the ICU.
The bulk of these lectures will be given during the first week of the rotation.
The remainder of the lectures will be given at varying times during the
rotation. Noon lectures are part of the resident’s core curriculum in internal
medicine and are elective for the medical student.
