It’s time to submit your profile for the UIC College of Medicine 2009 Reunion Alumni Annual.
It’s that time of year again — time to send us a current photo and updated profile for the UIC College of Medicine’s 2009 Reunion Alumni Annual.
Each year, our alumni write in from around the country and around the world with updates about their lives and their latest accomplishments, memories about their medical school days and sage (and sometimes surprising) advice for today’s UIC medical students. This information is then compiled into a book and distributed to all alumni in the Reunion classes.
Remember, our 2009 Reunion Alumni Annual won’t be complete without you. Please click here to download the PDF version of the form. You can then print it out and mail it back to us at:

Alumni may also email profiles and photos to reunion@uic.edu. The subject line of your profile email should read “2009 Reunion Alumni Annual Entry.” If you are emailing a photo, please send a JPG formatted file (in color or black and white) at a minimum size of 3”x 5” at 300 dpi. The subject line of your photo email should read “2009 Reunion Alumni Annual Photo.”
